Every person is unique and treatments are not Recipes. At Perfect Balance you spend your entire treatment with our licensed PT, giving you more bang for your buck. We are out of network, but will give you a discount, so you will not be precluded from coming here. We left all of the networking contracts because we were not able to spend quality time with our clients. We have the luxury of spending the full hour with you because we don’t use unlicensed support staff. The treatments include manual therapy, modalities, bio feedback, and the list goes on. People normally experience faster recovery and less visits overall. We are conscientious about optimizing treatment and educating you for as much home care as possible. We can schedule a time to talk if you have further questions. We hope to hear from you.
When you call to have us schedule your first appointment, please have your prescription (if applicable) and insurance card handy. This will allow us to confirm your insurance benefits and give you a coverage estimate.
To bring/keep in mind on your first visit:
1. Please wear comfortable and loose clothing that allow access to skin on body being evaluated.
2. Bring anything you would like your therapist to know in the form of medical or imaging reports or a MD prescription, if you have one. Please note: PTs DO NOT NEED A RX from an MD, in CA.
3. You will be emailed a Medical Health Questionnaire and questions about your function and pain. This should be completed prior to your Initial Evaluation to allow for complete Electronic Medical Records. Please answer all questions on the survey, even if it isn’t exactly clear to you. We can help address questions when you arrive in office.
4. The completion is required for proper documentation regulations and allows for higher reimbursement from the insurance company. Please look in your Spam folder if you do not see it.
Your insurance benefits will be determined before your first visit and you will be informed of what the amount of your deductible met and the percentage coverage you have. This is an approximate amount and not guaranteed. We are unable to obtain exact dollar amounts as that is at the discretion of your insurance company. We accept cash, check, debit card, and credit card. We are happy to discuss payment plan options as well. It is our policy that payment is made at the time of service. We will determine your Insurance company policy for out of network reimbursement. If allowed by your policy we are happy to electronically bill your insurance directly, and they will reimburse you directly with a check for the amount of PT that is allowed by them and the % of assistance per your policy. The alternative method used by insurance companies is obtained when you, the patient, directly submits a claim for each visit to your insurance portal. Perfect Balance will provide a Superbill/Claim”, upon request for submission to your insurance company for repayment. You will pay the full cost of treatment when you come and expect reimbursement via mail or after submission of claims. This is typically a 30-45 day process.
